Health and Medical Alerts

• Please provide the nurse with any health and medical concerns along with prescribed medications that may effect the health, safety and/ or academic performance of the student.

• Examples of health alerts to report are: ADD, ADHD, any allergy, asthma, diabetes, hearing concerns, vision impairments, cardiac conditions, epilepsy/seizures, fainting/syncope, surgical procedures, psychological disorders (such as depression, bipolar, schizophrenia, ODD, OCD)

• Please provide medical notes from the attending doctor with a diagnosis, medication requirements, if any and interventions required during the school day.

• The school nurse will follow-up with the family to discuss the needs of the student and determine what will need to be communicated to the necessary staff members to assure safety and academic support if needed.
